As we move through 2021, optimism abounds. While your practice and staff continue to face numerous and significant challenges during the pandemic, ACN continues to strengthen its support of your business and patient care through:

ACN’s new Provider Portal continues its integration process with practices into 2021. The new Portal offers providers a variety of patient-level information to support patient care, including the following for primary care providers and specialists: Primary Care Providers

  • Risk stratification patient reports
  • Annual Wellness Visit chart preparation and value-based arrangement (VBA) gap reports
  • View attributed patient population reports from ACN-affiliated payer partners
  • View a patient’s health conditions, medical care history, and other information
  • ADT alerts (facility, diagnosis, discharge date, disposition) for your assigned ACN patients
  • View your practice’s provider and location information on file with ACN
  • Update your practice’s hours of operation

Specialists

  • Patient Lookup tool includes a patient’s personal information, health insurance, and medical care history

A new user account is required to access the new Portal once it is available for your provider organization. Providers who haven’t done so already are strongly encouraged to request access to begin the process by March 31. After submission, ACN will contact you to complete the process when the new Portal is available for your provider organization. Please remember to request access and schedule your practice’s education by March 31 to meet 2021 PCP bonus program eligibility requirements. Please contact your Clinical Performance Consultant or email practicetransformation@azcarenetwork.org with any questions or support in accessing and using the new Portal.

This will help support the network in putting our performance in a favorable position for potential shared savings. ACN’s Quality Data Abstractors (QDA), Clinical Performance Consultants (CPC), and Care Coordination teams will assist through multiple touch points to help your practice identify members with specific care gaps and facilitate a visit with you. Quality focuses include:

• Hypertension (HTN) • Diabetes • Well Child Visits • Pain management • Prevention

Hypertension (HTN)Patient(s) 18-85 years of age with hypertension and most recent blood pressure less than 140/90 mm Hg in the last 12 months.
DiabetesPatient(s) 18 - 75 years of age that had annual screening for nephropathy or evidence of nephropathy.
Patient(s) 18 - 75 years of age with evidence of poor diabetic control, defined as the most recent HbA1c result value greater than 9.0%.
Patient(s) 18 - 75 years of age with most recent HbA1c result value less than 8.0%
Patient(s) 18 - 75 years of age that had a HbA1c test in last 12 reported months
Patient(s) with statin adherence (proportion of days covered) at least 80% during the treatment period.
Patient(s) 18 - 75 years of age that had an annual screening test for diabetic retinopathy.
Well Child VisitsPatient(s) 12 - 21 years of age that had one comprehensive well-care visit with a PCP or an OB/GYN in the last 12 reported months.
Patient(s) that had six or more well-child visits with a PCP during the first 15 months of life.
Patient(s) 3 - 6 years of age that had one well-child visit with a PCP in the last 12 reported months.
Pain ManagementPatient(s) 18 years of age or older with an average milligram morphine equivalent (MME) > 90 mg/day during the treatment period.
Patient(s) with uncomplicated low back pain that did not have imaging studies.
PreventionPatient(s) 52 - 74 years of age that had a screening mammogram in last 27 reported months.
Women 21–64 years of age who were screened for cervical cancer.
Patient(s) 50 - 75 years of age that had appropriate screening for colorectal cancer.
Patient(s) 16 - 24 years of age that had a chlamydia screening test in last 12 reported months.
Patient(s) 18 years of age and older who were screened for tobacco use one or more times within 24 months and who received tobacco cessation intervention if identified as a tobacco user
Patient(s) 12 to 17 years of age and adult patients 18 years of age or older with major depression or dysthymia who reached remission 12 months (+/- 60 days) after the first instance of a PHQ-9 greater than 9.
Patient(s) aged 12 years and older screened for depression on the date of the encounter or 14 days prior to the date of the encounter using an age appropriate standardized depression screening tool and if positive, a follow-up plan is documented on the date of the eligible encounter.
Patient(s) 65 years of age and older who were screened for future fall risk.
Patient(s) aged 6 months and older seen for a visit between October 1 and March 31 who received an influenza immunization.

  • ACN is proud to welcome Deepanshu Garg, MD, to its physician-led Board of Managers. Dr. Garg is affiliated with Med-Cure Internal Medicine in Goodyear and an ACN participant since 2016.
  • Dignity Health’s Center for Diabetes Management moved to its new location near the Loop 202 and Cooper Rd. in Gilbert, between Chandler Regional Medical Center and Mercy Gilbert Medical Center. The Center for Diabetes Management offers in-person and virtual educations services for all patients. Classes focused on Type 1 and Type 2 Diabetes, gestational diabetes, and pre-diabetes are available, along with Healthy Eating, Active Living (HEAL), a 24-week lifestyle change program taught by a health coach.
  • Abrazo Arrowhead Campus was named a Blue Distinction Center+® for its maternity services, and hip and knee replacement services by Blue Cross and Blue Shield of Arizona. The designation recognizes healthcare facilities and providers for quality and efficiency in delivering specialty care. Blue Distinction Center+ for Maternity Care designation is granted for meeting several quality measures and demonstrating better overall patient satisfaction for natural and cesarean section deliveries. Blue Distinction Center+ for Knee and Hip Replacement recognizes facilities that perform total knee and hip replacement surgeries safely, effectively, and cost-efficiently.
  • Dignity Health’s Electrophysiology Lab welcomed two new cardiologists. Dr. Jaskanwal "Sonny" Bisla is a cardiac electrophysiologist with Arizona Cardiac Electrophysiology PLLC, and Dignity Health Heart Arrhythmia Center at both Chandler Regional Medical Center and Mercy Gilbert Medical Center. He is board certified in Clinical Cardiac Electrophysiology, Cardiovascular Disease, and Internal Medicine. Osama T. Niazi, DO, is a cardiac electrophysiologist at Arizona Cardiac Electrophysiology PLLC, Dignity Health Heart Arrhythmia Center, Chandler Regional Medical Center and Mercy Gilbert Medical Center. Dr. Niazi is board certified in Internal Medicine, Cardiovascular Disease, Cardiac Echocardiography and Cardiac Electrophysiology.
  • Abrazo’s West Campus earned Primary Plus Stroke Center certification for its ability to evaluate, stabilize, and provide emergency care to all patients with acute stroke symptoms for possible admittance to a dedicated stroke care unit. The certification indicates Abrazo West Campus goes beyond the requirements of a primary stroke center, with the addition of training, equipment, experience, and personnel for performing thrombectomies and post-care for the treatment of acute ischemic strokes.
