In the first partnership of its kind in the healthcare industry, Arizona Care Network worked with partner Solve.Care to launch game-changing blockchain technology, with a promise of improving clinical outcomes and patient and provider satisfaction while addressing rising healthcare costs.
Phase 1: ACN Care.Wallet for Physicians
ACN has always focused on providing tools and resources to help providers do what they do best: administer care to patients to achieve the best possible clinical outcomes. Doctors spending so much time on administrative work are not spending quality time communicating with and touching their patients.
When we asked our providers what they need from ACN to improve care, they told us:
- Clear metrics, so we know what we need to do to be successful
- Data transparency, so we always know how we are performing against the standard
- Timely insights, so we can close care gaps to improve the health of our patients
- Accurate payment of the incentives and gain share
- But don’t increase our administrative burden
Our solution is the ACN Care.Wallet, a mobile digital wallet that delivers anytime access to metrics that matter, all right at hand on the doctor’s smart phone.
The ACN Care Wallet displays actionable insights that allow providers to proactively deliver better care. Within the Care.Wallet they can view their count of members that have clinical care gaps and other key metrics.
Data may drive population health, but care is delivered one patient at a time. If a provider sees on their ACN Care.Wallet performance card that 200 of their 1,000 Medicare patients have not had their annual wellness visit, they can proactively reach out to them to schedule the appointment. As they close care gaps, the blockchain updates the information.
Phase 2: Care.Wallet for Patients
Introducing the Diabetes Clinical Network
The Care Wallet for Physician was just the beginning. Now, our technology partner, Solve.Care, has announced a collaboration with Boehringer Ingelheim Pharmaceuticals, Inc., to launch a patient-centric care network on the Solve.Care platform. The Diabetes Care Administration Network aims to improve the well-being of patients with diabetes, and who may also be at an increased risk for cardiovascular disease.
Arizona Care Network patients will be the first to have access to the Diabetes Care Administration Network.
Despite recent medical advances, heart disease is still the number one cause of death for people with diabetes. Recent changes to treatment recommendations reflect the urgency in driving a fundamental change in how healthcare providers consider how they approach the care of people with type 2 diabetes.

The Diabetes Care Administration Network will help raise awareness about the diabetes and heart disease link, as well as encourage engagement between patients and providers to proactively address diabetes and associated cardiovascular complications. The network will use Care.Wallet, a proprietary tool created by Solve.Care, to support patients managing these dangerous and debilitating chronic conditions and help them optimize and coordinate care with their families and physicians.
ACN will provide concierge care management and support care coordinators, with the goal of achieving better healthcare outcomes for participating patients. Based on the results of this initial launch, expansion opportunities in the U.S. and globally will be evaluated.
