Advance Directives
Advance directives document your wishes for future medical care if you cannot speak for yourself — and help your doctors and family honor those wishes.
Why it matters
Discussing goals of care with your primary care doctor while you are well makes difficult decisions easier later. Directives can include a health care power of attorney and living will.
Talk with your PCP
Your primary care provider can help you understand options and point you to Arizona-specific forms. This is a conversation, not something Care Compass stores.
Keep copies accessible
Share copies with your care team, hospital, and trusted family members. Update directives when your wishes change.
