- Arizona Care Network, in partnership with Intel Connected Care, created the Care Scorecard app to help you track important annual preventative care visits, and provide resources and support to help achieve your best health. Register using your Connected care ID card today online, or download the Care Scorecard app in the Apple iOS or Google Play stores. The first 50 participants who register and complete 5 health tasks will receive a $50 gift card.
- In-network virtual visits are covered at 100% (deductible waived). Access the health care you need from wherever you are, whether through Intel’s Health for Life Centers, (OC-2 and CH-3) or within the Find a Provider tool, which offers a robust listing of ACN in-network providers.

Why Connected Care?
Both the Connected Care ACN High Deductible Health Plan (HDHP) and Primary Care Plus (PCP) are designed to improve your personal health care experience while focusing on preventative care and proactive management of existing conditions.
Care Coordination Services
ACN’s Care Coordination team gives you individual support from Registered Nurses, Navigators, Social Workers, and Behavioral Health Coaches who provide trustworthy health information and help you follow your doctor’s plan of care to achieve your best health.
ACN’s Care Coordination team can:
- Meet with you over the phone or in person
- Get the answers and resources you need
- Help you manage multiple medications so you don’t have negative interactions
- Coordinate healthcare appointments and even attend appointments with you if you wish
- Teach you tips for self-care
There is never a bill for Care Coordination services. ACN provides this support at no charge to the patient because keeping you healthy is important and reduces healthcare costs associated with worsening health and hospitalization.
